Gather your identification document (passport or residence permit) and the porting authorization code from the origin number you want to transfer.

Fill in the form below with the requested information, and in the end, a PDF document will be generated, which you have to print out and sign.

Attach photos of the completed and signed form and your identification document, and send them to the e-mail address manter@nos.pt
